Insurance
Your health insurance or employee benefit plan may cover services in full or in part. I accept some insurance plans in-network and will work with your out-of-network benefits.

In-network with FloridaBlue, Cigna, Aetna, Humana, Magellan and TriWest (Veteran Choice Program).

For your benefits, please check your coverage carefully by asking the following questions:

*Do I have mental health benefits?

*What is my deductible and has it been met?

*How many sessions per calendar year does my plan cover?

*How much does my plan cover for an out-of-network provider?

*What is the coverage amount per therapy session?

*Is approval required from my primary care physician?

**Therapy appointments by phone or internet video-conference are also available depending upon your insurance and need.  Please call for more information.

Employee Assistance Programs (EAP)

Life Navigation provides services for numerous Employee Assistance Programs that are a part of many employee benefits packages available through your employer.  In many cases these benefits may provide a limited number of cost free services.  Contact your human resource manager to find out if your employee benefits include this service.

Payment
Payment is expected at the time of service.  Cash, Zelle, Venmo, medical savings plans, and all major credit cards and debit cards are accepted for payment, with a 3% convenience fee on credit and debit card charges.

Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified me at least 24 hours in advance, you will be required to pay a $25 missed appointment fee.
